Contact me either by phone or email. You may also fill out my contact form and briefly describe your concerns and providing basic demographic information. I will respond within 24-48 hours.
Complete Forms
If you choose to schedule an appointment, I will email you forms to complete electronically prior to your first appointment. These forms may also include questionnaires to help me gain helpful information to make the most out of our first meeting.
Initial Meeting
The purpose of our initial or intake appointment (aside from having a chance to meet!) is for me to gather background information in order to better understand what type of evaluation process may be most suitable to you or your child. This appointment can be conducted either virtually or in-person.
